• Pearls

  • Anterior canaloplasty

    • Work lateral to medial

    • Maintain a medial based skin flap

      • Remember "Skin is gold"

      • Protect the sky flap with foil

    • Drill lateral to medial with 3-4 diamond; use copious irrigation to limit heat damage

    • Curette the medial ledge

  • Posterior canaloplasty

    • Be more conservative

    • Avoid the mastoid air cells

    • Be aware of an anterior facial nerve; use the monitor

  • Atticotomy

    • May extend to expose the epitympanum; separate IS joint and tensor tympani

    • May lateralize the malleus and incus and reconnect with Otomimix
