Considerations

• • 2007: Transoral laser microsurgery for untreated glottic carcinoma (Hinni, Hayden, Perry) 

Preparation

• • See TLM 1 - Setup for photos and instructions

  • See MDL - Carcinoma for more on considerations and technique

• Smallest laser safe ETT that can be used

  • • ETT to left lower lip

• • Head on foam donut, no shoulder roll

  • Turn bed 90 degrees counter clockwise

  • Place thermasplint tooth guard

    • Heat in hot water to make malleable, place on teeth, cool with ice water gauze

  • Split sheet tucked under shoulders, head drape

  • Laser 

Procedure

• • Expose glottis and suspend

  • Take pictures with Hopkins rod

  • Bring microscope in
